Immigration Physicals
OccMedCIC has a seasoned United States Citizenship and Immigration Services (USCIS) Civil Surgeon on staff. Our office will complete an I-693 form and provide it to the patient to submit to the USCIS.
Lab results, a copy of the completed form, and the sealed original forms are available within one week. If any vaccine information is missing, our office is able to provide immunization services to you as needed.
What to Expect
There is no need to fast before the immigration physical. Please enjoy normal food and drink before the appointment. The appointment will consist of:
- An interview with the Civil Surgeon to discuss vaccinations and medical history
- A brief physical exam
- Blood tests and urine collection for lab testing. There is no need to fast prior to this exam.
Included in the cost of the appointment is a blood test for tuberculosis for those 5 years and older.
Blood test for syphilis (RPR) and urine tests for gonorrhea and chlamydia will be added if required, based on current immigration physical exam guidelines.

What to Bring
- USCIS Letter (if you have it)
- Vaccination or immunization record: See USCIS vaccination requirements for the complete list. For those 18 and older, vaccines typically include: COVID (documentation of all doses), flu (current year), Td or Tdap (within 10 years), MMR (two doses or titers for those born after 1956), Varicella (two doses, titers, or history), Hepatitis B (three doses or positive antibody titer).
- Vaccine schedules: U.S. Recommended Vaccination Schedule
- Government-issued photo ID: Passport or U.S. driver’s license.
- Payment: We do not accept cash or insurance for the exam. Please bring a credit or debit card. We will provide an itemized receipt to facilitate reimbursement by insurance.
Cost of the Exam
Immigration Physical Exam: $485 per exam.
Our cost includes the required tests associated with the Immigration Physical Exam. Any additional vaccinations or blood tests are an additional cost and can be reviewed with the provider.
